WEEKLY KABBALAT SHABBAT SERVICE REGISTRATION
We are so thrilled you want to join us at this Friday's Kabbalat Shabbat service!

PLEASE NOTE THE FOLLOWING:

* In order to keep everyone healthy and employ social distancing, Midway's Shabbat services will have limited attendance allowed.  Please complete this form to register.  

* Registration opens each week at noon on Sunday, and closes at noon on Friday or when the maximum amount of attendees have registered, whichever comes first.

* We are happy to welcome you with your children.  Children are expected to be with their parent/guardian at all times.  No children are to be by themselves in the hallways or other parts of the synagogue building.  

* We will contact you by phone or e-mail by Friday at 2:00PM to confirm your attendance at the Kabbalat Shabbat service.
IMPORTANT CONSIDERATIONS IN ORDER TO REGISTER:
In the last two weeks, did you care for or have closed contact with someone diagnosed with COVID-19?
IF THE ANSWER IS YES, PLEASE DO NOT ATTEND SERVICES
In the past 14 days, have you traveled internationally, or returned from a state/territory identified by New York State as having widespread community transmission of COVID-19? (this does not include passing through a state for less than 24 hours).  Please click here for the current list of states/territories requiring quarantine.
IF THE ANSWER IS YES, PLEASE DO NOT ATTEND SERVICES
BE ADVISED:

- Temperature checks will be required to enter the building, and you will be asked to wear a mask / face covering

- Hand sanitizer is provided and must be used upon entering and leaving both the building and the sanctuary

- To protect the health of everyone in our Midway community, we ask that you do not attend services if you are not feeling well
